What is a medical record?
A medical record is a history of someone’s health. As a parent, you have a right to access your child's medical records. Learn more about what medical records.
You may request your child's medical records (for a child younger than 18 years of age) from the Health Information Management Department by completing the linked authorization form:
This completed, signed, and dated form may be submitted via fax, email or mail along with a clear picture of your valid Photo ID/Driver's License.
If you are requesting medical records about a patient other than your own child, please send the authorizing documents such as power of attorney, guardianship papers, or other documentation that shows appointment as legal representative.

Medical Records is in the Health Information Management Department on the 6th Floor of the North Tower. Hours are Monday through Friday between 8 a.m. and 4 p.m. if you would like to request medical records in person.
