Transfer Center
The Transfer Center (Children’s Connect) is responsible for coordinating several critical functions, including managing patient referrals to the hospital, overseeing bed placement, handling direct admissions, addressing after-hours calls, and conducting organizational follow-up calls.
Specifically, the Transfer Center ensures that:
- Patient Referrals to ETCH: All incoming patient referrals are efficiently managed, ensuring patients are directed to the appropriate specialized units. This process also includes the activation of pediatric and neonatal transport when necessary, ensuring timely and safe transfer to the required care facilities.
- Bed Placement: The Transfer Center coordinates the placement of patients in hospital beds, optimizing capacity and ensuring that patients are assigned to the most suitable care areas. Additionally, we manage bed control by reviewing blocked rooms and monitoring patients whose length of stay exceeds the recommended timeframe, ensuring efficient use of hospital resources and timely patient flow.
- Direct Admissions: The Transfer Center streamlines the direct admission process, efficiently managing the intake of patients from various entry points, such as physician referrals, urgent care centers, or emergency services, to ensure a smooth and timely transition into the hospital system.
- After-Hours Calls: The Transfer Center manages after-hours calls from pediatrician offices, offering guidance on a range of care options—from at-home care instructions to advising when to call 9-1-1 for emergency transfers to the hospital. This ensures that urgent or time-sensitive patient needs are promptly addressed, even outside regular office hours, facilitating timely and appropriate care.
- Follow-Up Calls: These calls are an automated call to discharged patient families. These calls are to help assess the patient's progress, address any concerns, and offer further assistance if needed. The goal is to ensure the patient is doing well, reduce the likelihood of readmission, improve patient satisfaction, and ensure a smooth transition back to home or another care setting.
By managing these processes, the Transfer Center plays a vital role in enhancing hospital efficiency, improving patient flow, and ensuring that patients receive timely, appropriate care.
